Résumé
Introduction : L’entorse de cheville représente 6 000 cas par jour en France. Elle représente la moitié des entorses ainsi que 20% des traumatismes du sportif. De plus, l’incidence de l’entorse de cheville représente 6% des traumatismes aux Urgences.
Objectif : Montrer une diminution de la douleur en associant l’ostéopathie à la prise en charge actuelle au sein du service des urgences.
Matériel et méthode : Les patients ont été recrutés au sein du service des urgences de Poissy (78). Les 14 patients vus par un médecin urgentiste ont ensuite été randomisés en deux groupes : l’un recevant un traitement ostéopathique, le second du Ligth Touch. Le critère de jugement principal étant l’EVA et le secondaire étant le volume de l’œdème de cheville.
Résultats : Pour l’EVA, nous avons obtenus un p > 0.05 entre les deux groupes et un p < 0.05 sur l’évolution à une semaine pour les deux groupes. Par ailleurs, pour le volume de l’œdème de cheville, nous avons obtenus un p < 0.05 seulement pour le groupe traité en ostéopathie à une semaine, ainsi qu’un p < 0.05 entre les deux groupes.
Conclusion : Pour l’EVA nous n’avons pas pu mettre en évidence de différence significative entre les deux groupes. Ce qui n’est pas le cas pour le volume de l’œdème, puisque nous avons obtenus des résultats montrant une amélioration plus importante pour le groupe traité en ostéopathie. Le travail mécanique réalisé par l’ostéopathe aurait permis d’améliorer ce critère. En améliorant la puissance et en optimisant la prise en charge ostéopathique, en utilisant les données du SOAP Note Form, une nouvelle étude serait susceptible de pouvoir montrer de meilleurs résultats.

Abstract
Title : Osteopathic care on patients admited in emergency for lateral sprain of the ankle. Experimental study.
Introduction : Sprained ankle represents 6000 cases each day in France. It represents half of the spains and 20 percent of the sports injuries. Moreover the incidence of sprained ankle represents 6 percent of the emergency trauma.
Aim : The goal of this study is to show a decrease of the pain by associating an ostepathic treatment added to the actual care in the emergency service.
Materiels and methods : Patients have been recruted in the emergency service of Poissy (78). The 14 patients who were seen by the doctors have been randomized in two groups : one receiving the osteopathic treatment and the other the light touch. To measure the intensity of the pain we will use the NRS, Numerical Rating Scale, and we will also measure the volume of the edema of the ankle.
Results : We got a p> 0,05 for the NRS between the two groups and a p< 0,05 one week later for both of the groups. Furthermore we obtained a p < 0,05 for the volume of the edema in the group treated by osteopathy one week later and a p < 0,05 between the two groups.
Conclusion : For the NRS we couldn’t put in evidence a significative difference between the two groups. In the contrary, the results show that there is a better improvement of the volume of the edema in the group treated by osteopathy. The mechanical work of osteopathy could have improve this critary. Improving the power and optimising the osteopathic care by using the SOAP note form could lead to a new study and show better reults.
