Résumé
Titre : Étude comparative sur le traitement des symptômes du spondylolisthésis lombaire : Traitement ostéopathique / chirurgie par arthrodèse antérieure lombaire.
Objectif : Le spondylolisthésis lombaire concerne environ 4% de la population. Le but de ce mémoire est d’évaluer l’efficacité de ces deux traitements.
Matériel et Méthode :
- La chirurgie par arthrodèse antérieure lombaire est étudiée dans ce mémoire à partir de 26 patients suivis par le Docteur PENCALET. Les analyses des données radiographiques ont été effectuées sur le site KEOPS. Nous avons étudié les paramètres de l’équilibre sagittal basés sur le rachis et sur le pelvis. Les parties « Résultats » et « Discussion » détaillent cette analyse.
- Le traitement ostéopathique a été pratiqué par Alice MARSAULT sur une patiente de la clinique « A Mains Nues » à Saint-Denis, souffrant d’un spondylolisthésis sur l’étage L5/S1. Nous avons uniquement utilisé les informations données dans ce mémoire de fin d’étude pour analyser l’efficacité du traitement ostéopathique afin d’étayer notre conclusion.
Résultats : Ils correspondent à l’analyse radiographique des patients ayant opté pour la chirurgie. Nous avons divisé les patients en trois groupes selon l’évolution de leurs paramètres de l’équilibre et leur impotence fonctionnelle dans la vie courante à l’aide du questionnaire OSWESTRY.I
Discussion : Nous avons observé après l’analyse des résultats des patients ayant subi la chirurgie, une diminution significative de la symptomatologie du spondylolisthésis L5/S1 ainsi qu’une amélioration de la qualité de vie pour la majorité d’entre eux. De plus, l’équilibre sagittal a été modifié permettant, pour beaucoup de patients, le rétablissement de celui-ci malgré des adaptations différentes.
Conclusion : si la chirurgie par arthrodèse lombaire antérieure et le traitement ostéopathique ont tous deux démontré leur effet bénéfique sur la diminution des symptômes du spondylolisthésis, on peut conclure que l’ostéopathie est une médecine manuelle adaptée permettant au patient de ne pas avoir à supporter un suivi post opératoire avec des traitements lourds contrairement à la chirurgie.
Abstract
Title : Comparative study on the treatment of the symptoms of the spondylolisthésis lumbar vertebra : Osteopathic treatment / surgery by previous arthrodese lumbar vertebra.
Objectives : The spondylolisthesis lumbar vertebra concerns approximately 4 % of the French population. The purpose of this report is to estimate the efficiency of these two treatments.
Materials and Methods :
- The surgery by previous arthrodese lumbar vertebra is studied in this report on 26 patients followed by Doctor PENCALET. The radiographic data analyses were made on the website KEOPS. We studied the parameters of the sagittal balance based on the rachis and on the pelvis. The parts "Results" and "The discussion" detail this analysis.
- The osteopathic treatment was practised by Alice MARSAULT on a patient of the "A mains nues" dispensary in Saint-Denis suffriing from a spondylolisthesis on the floor L5 / S1. We only used the information given in this report to analyze the efficiency of the osteopathic treatment to support our conclusion.
Results : It represent the radiographic analysis of the patients who chose the surgery. We divided the patients into three groups according to the evolution of their parameters, of the balance and their functional disability in the common life thanks to the questionnaire OSWESTRY.
Discussion : With the analys of the results of the operated’ patients, we can note a significant decrease of the pain due to the spondylolisthesis L5 / S1 as well as an improvement of the quality of life for the majority of those. By the way, the sagittal balance was changed for many patients with different adaptations.
Conclusion : If the surgery by anterior arthrodese lumbar vertebra and the osteopathic treatment both demonstrated their beneficial effect on the decrease of the symptoms of the spondylolisthesis, we can conclude that the osteopathy is an adapted manual medicine allowing the patient not to support a follow-up operating with heavy treatments contrary to the surgery.
