Efficacité d’une prise en charge ostéopathique sur des patients atteints du syndrome du canal carpien

Résumé

Introduction : Le syndrome du canal carpien est un trouble musculo-squelettique très fréquent chez les travailleurs manuels, provoquant une charge financière élevée pour la société. La chirurgie reste le seul remède pour les cas modérés à sévères. Les traitements manuels tendant néanmoins à faire leurs preuves, nous nous sommes donc intéressés à l’efficacité de la prise en charge ostéopathique globale des patients diagnostiqués par électromyogramme.

Méthode : 12 sujets répartis aléatoirement dans deux groupes, un groupe traité en ostéopathie (n = 7) et un groupe Light Touch (n = 5), ont été sélectionnés afin de bénéficier de trois consultations durant un mois. Le Boston Carpal Tunnel Questionnaire nous a permis d’évaluer la sévérité des symptômes et l’état fonctionnel du patient et le SOAP Note Form nous a permis d’étudier l’évolution des dysfonctions somatiques (DS) des patients.

Résultats : L’analyse des mesures a été statistiquement significative en ce qui concerne l’atteinte globale (p = 0,02) et la sévérité des symptômes (p = 0,02). En revanche, malgré une nette amélioration (p = 0,06), l’état fonctionnel n’a pas obtenu d’amélioration significative. L’étude du SOAP nous a permis d’observer que les DS liées aux zones cervicales, plexus brachial, avant-bras, poignet et main ont nettement diminué dans le groupe traité par rapport au groupe Light Touch.

Conclusion : Cette étude a montré que la prise en charge ostéopathique globale favorisait la diminution des douleurs et l’amélioration de l’état fonctionnel des patients atteints du syndrome du canal carpien dans les cas légers à modérés. Néanmoins, il serait nécessaire de poursuivre cette étude en agrandissant l’échantillon ainsi qu’en allongeant la période de prise en charge et de suivi des patients afin de tenter d’obtenir des résultats significatifs sur le long terme.

Title : Effectiveness of osteopathic manipulative treatment in patients with carpal tunnel syndrome. Experimental study.

Introduction : Carpal tunnel syndrome is a very common musculoskeletal disorder amongst manual workers, causing a high financial burden on society. Surgery remains the only remedy for moderate to severe cases. However, manual treatments tend to prove their worth, so we have focused on the effectiveness of the overall osteopathic treatment of patients diagnosed by electromyography.

Methods : 12 subjects randomly assigned to two groups, one group treated with osteopathy (n = 7) and one Light Touch group (n = 5) were selected for three consultations during one month. The Boston Carpal Tunnel Questionnaire allowed us to evaluate the severity of the symptoms and the functional state of the patient and the SOAP Note Form allowed us to study the evolution of the somatic dysfunctions (SD) of the patients.

Results : Measurement analysis was statistically significant for overall impairment (p = 0,02) and severity of symptoms
(p = 0,02). On the other hand, despite a marked improvement (p = 0,06), the functional state did not obtain any significant improvement. The study of the SOAP allowed us to observe that SD related to the cervical, brachial plexus, forearm, wrist and hand areas decreased significantly in the treated group compared to the Light Touch group.

Conclusion : This study showed that overall osteopathic treatment favored the reduction of pain and improvement in the functional status of patients with carpal tunnel syndrome in mild to moderate cases. Nevertheless, it would be necessary to continue this study by extending the sample and extending the period of care and follow-up of patients in order to try to obtain significant results over the long term.
