Résumé
Introduction : Les douleurs neuropathiques possèdent plusieurs étiologies et représentent une forte prévalence au sein de la société. Elles sont principalement traitées par une approche allopathique associée à de la kinésithérapie. Le traitement actuel n’intègre pas l’ostéopathie. De plus peu d’études ont été réalisées sur la prise en charge ostéopathique de cette douleur. Objectif : nous cherchons à évaluer l’effet de l’ostéopathie concernant l’évolution de la symptomatologie des sujets atteints de neuropathies diabétiques ainsi que l’amélioration possible de leur qualité de vie.
Méthode : Recrutement de dix-sept patients diabétiques atteints de douleurs neuropathiques. Etude randomisée avec une répartition des patients en deux groupes : le groupe 1 est traité en ostéopathie et le groupe 2 est un groupe témoin. Nous avons ensuite recueilli les résultats du DN4 (critère de jugement principal) et de l’EVA pour nos 2 groupes après explication des différents questionnaires. Enfin, nous avons décidé des différentes dates d’appel pour le groupe témoin afin de correspondre au schéma de consultation du groupe traité.
Résultats : Il existe une différence statistique significative entre nos deux groupes concernant le DN4 après trois consultations d’ostéopathie. Les patients du groupe traité présentent une diminution de leurs symptômes par rapport au groupe témoin.
Conclusion : La prise en charge ostéopathique aurait une influence positive sur la symptomatologie des patients diabétiques sujets à des douleurs neuropathiques. L’ostéopathie pourrait donc avoir sa place dans une équipe pluridisciplinaire pour prendre en charge ces patients. Cependant, Il serait intéressant de poursuivre ces études pour pouvoir accéder à un niveau de preuve supérieur.

Abstract
Title : Influence of osteopathic care upon symptomatology of diabetic patients suffering from neuropathic pain (clinical case study)
Introduction : Neuropathic pain may have various etiologies and exhibit an important prevalence within society. Neuropathic pain is mostly treated by an allopathic approach associated with physiotherapy Current treatment does not include osteopathy. Moreover, few studies have been devoted to osteopathic care of this symptomatology. Objective : we seek to evaluate the osteopathic effects concerning the evolution of the symptomatology of the subjects reached of neuropathy diabetic as well as the possible improvement of their quality of life
Method : Recruitment of seventeen diabetic patients suffering from neuropathic pain. Randomized study with distribution of patients in two groups, the first one being treated by osteopathy and the second one being the control group. Then, we collected the DN4 result (main criterion of judgements) and of the EVA four our two groups after explanations of the different questionnaires. Lastly, we decided various call dates for the reference group in order to correspond to the diagram of consultation of the treated group.
Results : We observed a statistically significant difference between the two groups with respect to DN4 after three appointments. The patients of the treated group present a reduction in their symptom compared to the reference group.
Conclusion : Osteopathic care might have an influence upon symptomatology of diabetic patients suffering from neuropatic pain. Osteopathy might therefore have its place in a multidisciplinary team taking these patients in charge. However, i twill be interested to continue these studies to be able to reach a higher level of evidence.
